RN Case Manager: Home Health (Full Time)

Westminster-Canterbury on Chesapeake Bay is currently hiring a Full Time Registered Nurse (RN) Home Health Case Manager for Westminster-Canterbury At Home. 

Westminster-Canterbury At Home offers personalized and exceptional Home Health Care, Home Care, and Hospice services in private home settings in Virginia Beach and surrounding areas. 

What will I do?

Our RN Home Health Case Manager:

• Works independently with groups of clients to provide initial and on-going assessment of the client's skilled needs.
• Implements individual plans of care and make referrals to others disciplines and /or revise the plan as necessary.
• Provides on-going consultation and education for patient/family member regarding all aspects of care.
• Communicates and collaborates with family/caregivers and other professionals involved in the care of the client.

What do I need for this role?

• Current, unencumbered Registered Nurse (RN) license in the State of VA (or compact license).
• One year of nursing experience, preferably in hospice, home health, or a skilled nursing facility.
• Current CPR Certification.
• Prefer WOCN (Wound, Ostomy and Continence Nurse).
• Current valid driver's license and proof of motor vehicle insurance required.
• Willing to drive to private homes in greater Virginia Beach area to perform the requirements associated with this position.
